Rehabilitation Case Management

Rehabilitation case management involves developing, implementing and completing a rehabilitation plan for our clients. We arrange the best treatment for that client, targeted towards agreed goals and expected outcomes. We will support the injured party and their families utilising medical and non-medical input. We evaluate progress, adjust strategies accordingly and review the goals regularly.

Providing Support

Where appropriate we include the whole interdisciplinary team. We support clients to access care whether the provider is NHS, local authority or the private sector and we facilitate communication between providers. Where current provision is inappropriate or inadequate, we support our clients to access alternative services.

Working with Rehab Managers

All our rehab managers are registered with their respective professional bodies. The team delivers a caring empathetic service putting the clients’ needs at the centre of the process. We work with our clients to improve function, reduce dependency and to support them to become fully involved in their rehabilitation.
